- Liability/Automobile Insurance of $1,000,000 (NHSIE must be added as an additional insured)
- Federal Taxpayer Information Form (W-9)
- Contractor and Subcontractor Demographic Information
- EPA Renovator Certification
- City Business license where you are awarded the job
- Before and after photos of recent projects with addresses
